Low income application form

To apply for the low-income fee as part of your registration or restoration application for the year ending 31 March 2022 complete the form below.

If you are applying to have your name entered on the register of optometrist or  dispensing optician for the first time or you are applying to restore to the register, and all of your gross personal income for the year 1 April 2021 - 31 March 2022 is less than £16,000 then you are eligible to apply for the low income fee. The low income rate is not available to bodies corporate or students. The online application must be submitted as part of your registration/restoration application and this must be indicated on your form.

Gross personal income includes all income from all sources worldwide. It does not only relate to income from optometry or dispensing optics related activities. It includes, for example, pensions, investments, maternity pay, sick pay, and any kind of benefit.

Will my application be audited?

If you elect to pay the low income fee you must, upon request, provide the GOC with copies of your payslips, bank statements or any other document related to your gross personal income. If your income is assessed as greater than £16,000 or you do not supply evidence, you must immediately pay the difference (£100). We may request this information at any time.

The GOC Standards of Practice include requirements around honesty, trustworthiness and professionalism. Making a false declaration or failing to inform the GOC of changes in your financial circumstances may call into question your fitness to practise.

Fees to pay

If you are successful for the low income fee, you will pay the relevant registration or restoration fee plus the pro-rata low income retention fee for the 2021/2022 registration.

The full year low income retention fee is £260. You will be charged based on how many quarters of the year you are registered for.

Please note that we are not able to retrospectively apply the low income fee rate to applicants who subsequently earn below the £16,000 threshold during the 2021-22 registration year.

If you have any questions about the low income fee and your eligibility please contact us at or on 020 7580 3898 (option 1).

How do I apply?

To apply please fill in this online form (do not use this form as part of your renewal application). We will contact you by email if your application has been successful or not.
